If  Shaping Your Future Psych services, are not covered under your insurance plan.  Full payment is required before your appointment, credit cards or cash are the only forms of payment accepted. It is the patient’s responsibility to request a detailed billing receipt to submit an out-of-network claim.  The patient is responsible for verifying out-of-network reimbursement with their insurance company.

• Self / Cash Payment: Payment is due at the time of service.
• Cancellations: Appointments must be canceled 24 hours in advance. Missed appointments without a call or portal message will incur an $85 fee. Patients who miss two appointments will be dismissed from the practice.
• Collections: Accounts not paid after 30 days will be referred to a collection agency, incur a 30% service fee, and be dismissed from the practice.
• Prescription refills:   Prescription refill requests following a missed appointment will incur a $55 fee.
• Requests for paperwork: Fees for paperwork requests vary depending on the records requested.
• Disability: We cannot perform assessments or complete paperwork for disability claims.
• Phone calls outside of appointment: Phone consults of more than 20 minutes will incur a $50 fee.
• HIPAA Notice of Policies and Practices: The Health Insurance Portability and Accountability Act and Virginia state law require that we protect the privacy of your personal information. For more information, click here.
